Measurement of Serum human Chorionic Gonadotropin and its Isomers, Free ¥â-subunit and Urinary Gonadotropin Fragment Levels in Patients with Gestational Trophoblastic Disease


Abstract
To evaluate the value of serum hCG, its free ¥â-subunit and urinary gonadotropin fragment(UGF) for early differentiation of gestational trophoblastic disease(GTD) from normal pregnancy and possible prediction for metastatic GTD, serum levels of
total
¥â-hCG and its free ¥â-subunit and urine levels of UGF were measured in 15 normal early pregnant women and 36 patients with GTD before treatment and during follow-up. The ratio(R) of free ¥â-subunit to total ¥â-hCG(ng/1000mIU) were compared in
each
groups-normal pregnancy(NP), molar pregnancy(MP), non-metastatic(NMGTD) and metastatic GTD(MGTD). Compared to NP group(R=1.12), the mean ratio of pretreatment samples of MP(R=3.03), NMGTD(R=2.95) and MGTD(R=6.36) groups were significantly
higher(p<0.001). The mean ratio of MGTD group was significantly higher than those of MP and NMGTD groups(p<0.05). However the ratio of serial samples during treatment follow-up were very inconstant. The ratio of UGF to total ¥âhCG were
insignificant
because UGF levels were very variable in same group.
These data suggest that excessive production of free ¥â-subunit of hCG may discriminate molar pregnancy from normal early pregnancy and predict a high likelihood of developing metastasis in patients with GTD. However, inconstant ratio of free
¥âto
total
¥âhCG during treatment follow-up limit its value for the clinical efficacy.
